THIS COMMUNITY CONSORTIUMS FIRST COMMUNITY EFFORT IS THE HOUSING OF THE
MENTALLY ILL, AND HOMELESS .

Our  grant proposal is for the purchase of a facility  to be called " Millie's"  place and will provide housing and utilities for 1 year..

We ask for no other funding for this project then the purchase of the existing building and the estimated costs of utilities for 1year.  Located in Downtown Martinsburg.


All support services for this project will be supplied by the member organizations of the consortium, as volunteers.
                                                                             
                                      
LIivings Skills Programs, from consumer workshop programs, with Appeers input on a consultant basis.

Transportation facility by Veterans on the Rise, and the Panhandle Cultural Diversity Coalition

Weekly supportive therapy group will be provided by Cathy Combs, LICSW and certified Addictions counselor of the Tri-County Professional Volunteers Program
                 
Crisis Health Services:  provided by Eastridge Health Systems
 

House Management : PHILIPP SPENCER WHO HAS MANANGED THE SUPPORTIVE HOUSING
VOTR PROGRAM FOR 2 FACILITIES FROM 1/01 -CURRENT.

THIS PURPOSAL IS UNIQUE IN THAT ALL SERVICES NEEDED BY CONSUMERS HOUSED IN
THE PURPOSED FACILITY WILL BE PROVIDED ON A VOLUNTEER BASIS FOR A PERIOD OF 3
YEARS . THIS WILL  BE A BIG DECREASE IN FUNDING BECAUSE NO SALARIES WILL NEED
TO BE FUNDED.
                                           
 IT IS OUR BELIEF THAT THIS TYPE OF "SHARE CARE HOUSING" WILL ALSO DECREASE
THE NUMBER OF NEEDED HOSPITALIZATIONS   WHICH HAS BEEN A LONG TERM GOAL OF
LOCAL PROVIDERS IN THE PANHANDLE.
                                                                             
                                             DURING THE COURSE OF THE
VOLUNTEER PERIOD OF 3 YEARS CONSUMERS WILL BE GIVEN THE TRAINING TO TAKE OVER
THE MANAGEMENT OF THE FACILITY.

UPKEEP AND OTHER EXPENCES FOR THE PURPOSED HOUSING

WILL BE PAID FOR FROM THE RENTS EACH RESIDENT WILL PAY TO LIVE THERE. THE
BUILDING WILL HOUSE 5 CONSUMERS, WITH 1 CONSUMER RESIDENT PROVIDING ON SITE
SUPERVISION AS NEEDED.

DAILY CASE LOGS WILL BE KEPT ON EACH RESIDENT.

A ADVISORY BOARD OF 5 MEMBERS HAS BEEN ESTABLISHED .

FROM THE RESIDENCE, 1 MEMBER APEERS MH CONSUMERS ORG.  1 MEMBER PANHANDLE

THE FOLLOWING ORGS. 1 MEMBER WV MENTAL HEALTH PLANNING COUNCIL, 1 MEMBER WV
ADVOCATES.

HOLIDAY THE HEAD OF
THE NAMI AFFILIATE IN MARTINSBURG.
